Every year, millions of people are infected with diseases you've probably never heard

of.

There's tons of media coverage about things like malaria, Zika, and HIV — all of which

pose serious threats to public health.

But there are many more diseases out there that are just as deadly -- yet not as infamous.

They're called neglected tropical diseases, or NTDs, because historically they haven't

been paid as much attention as other diseases, and they mainly affect people in tropical

areas.

But these days, global healthcare initiatives are paying attention to them -- working to

find ways to prevent and treat these diseases.

The deadliest NTD — caused by the second-deadliest parasite in the world, after malaria's — is

schistosomiasis, also known as snail fever.

It's caused by a parasitic worm, called schistosoma, that's transmitted through

water that's been contaminated with the parasite's eggs.

The parasites use certain kinds of snails in the water as hosts, to grow and multiply.

Eventually, the parasites' larvae are released into the water, and that's when anyone who

comes in contact with the water is at risk, because the larvae can burrow through the

skin.

Infected people often don't have symptoms for the first month or two, though they can

sometimes end up with a rash or other symptoms like fever, aches, and a cough.

Symptoms are much more common later on, once the larvae have grown into adults and laid

eggs.

That's when the infection is considered chronic.

But the symptoms of chronic schistosomiasis aren't caused by the full-grown worms — the

worms can actually incorporate proteins from the host to trick the immune system into ignoring

them.

It's the eggs that cause an incredibly strong immune response, which can strike almost anywhere:

the intestines, the lungs, the bloodstream, even the brain.

So schistosomiasis can cause all kinds of symptoms, from diarrhea to wheezing to seizures.

More than 200 million people have schistosomiasis right now, and about 200,000 people die from

it every year.

The main way to treat it is to prevent it: if people are regularly given a drug called

praziquantel, even if they don't have the disease, they're less likely to develop

a severe infection later on.

But the best way to protect people in the long term is to make sure communities have

access to sanitation and safe drinking water, to stop the parasite from spreading.

Another neglected tropical disease is Chagas disease.

It's most prevalent in Central and South America, where about 8 million people have

the infection, and about 10,000 die from it every year.

Chagas is caused by a parasite called Trypanosoma cruzi, a single-celled protozoan.

The parasite spreads through the feces of infected insects, often known as kissing bugs.

Usually, the parasite enters the body through mucous membranes — like in the eyes and

mouth — or through broken skin, when someone scratches a kissing bug bite.

The best time to treat the disease is just a few days after infection, but most people

don't show any symptoms for weeks.

And even in those who do, the symptoms are pretty mild and nonspecific — like fever,

a rash, and diarrhea.

And then something frightening happens: After this initial phase, Chagas infection usually

goes dormant -- for years.

After as much as a decade or two, about 30% of infected people then develop chronic, life-threatening

symptoms — including swelling of the heart, or the intestinal tract.

So, prevention and early detection are very important here.

But as with other NTDs, it often affects people who don't have easy access to healthcare.

Fortunately, efforts are underway to help those at risk, mainly with the help of insecticides.

If you kill the bugs, they can't spread the disease.

Then there's African trypanosomiasis, otherwise known as African sleeping sickness, which

affects parts of sub-Saharan Africa.

Like Chagas, this disease is caused by parasites carried by insects — in this case, by the

bite of the tsetse fly.

In the first phase of sleeping sickness, the parasite reaches the peripheral nervous system

— the nerves that aren't part of the brain or spinal cord.

Here, symptoms include things like fever, joint pain, and itching, which can be caused

by lots of different things.

So — again like Chagas — sleeping sickness is hard to diagnose at first.

But symptoms become more severe when the parasite invades the central nervous system — the

brain and spinal cord.

Then, it can cause convulsions, confusion, and the symptom that gives the disease its

name: uncontrollable sleepiness.

If left untreated, sleeping sickness can be fatal.

But it is treatable with combined medications, even in the second stage.

And initiatives to treat sleeping sickness have become more and more successful in recent

decades.

In 1995, there were an estimated 300,000 new cases of African sleeping sickness.

But since then, health organizations have been working on testing people for the disease

and getting treatment to those who need it.

They've also been using insecticides to kill tsetse flies.

By 2014, there were fewer than 15,000 new cases of sleeping sickness.

There are lots of other NTDs out there as well.

But they aren't being neglected anymore, either, and that's making a big difference.

Du Bois & Race Conflict: Crash Course Sociology #7 - Duration: 10:40.

Two bachelor degrees. PhD from Harvard University.

Two-year fellowship to study in Berlin.

Professor of sociology and history at two different universities.

Author of countless books.

Activist and co-founder of a key civil rights organization.

Editor and co-founder of a magazine.

And a poet to boot. Pretty good resume, yeah?

What if I make it a bit more impressive?

That PhD from Harvard?

First Harvard PhD granted to an African American.

The civil rights organization?

The NAACP.

That magazine?

The Crisis, the longest running black publication in the United States, in print since 1910.

This resume belongs to William Edward Burghardt DuBois, whom you might know better as W.E.B. Dubois.

He was one of the earliest American sociologists, as well as one of the first proponents of race-conflict theory.

And his studies of the lives of African Americans during the Jim Crow era of American history

– and the oppression they faced – are the cornerstones of how sociologists study race.

W.E.B. Dubois was born in a small town in Massachusetts in 1868.

1868 – that's five years after the Emancipation Proclamation.

Three years after the end of the American Civil War.

And the same year that the 14th amendment was passed.

At this time, race was considered a biological construct.

Slavery, and later the Jim Crow laws – laws in the South that enforced racial segregation

– were framed as natural consequences of the supposed, natural inferiority of Blacks to Whites.

We, of course, now know that this was not just wrong, but deeply harmful.

And more than that – the idea that race itself is a purely biological, immutable quality is also understood today as being simply untrue.

Instead, race is thought of as a socially constructed category of people, who share biological traits that society has deemed important.

Yes, human beings vary a lot in how we look – our skin color, our facial features, our body shapes, our hair texture.

But those visual markers only become a "race" when members of society decide that specific markers constitute a specific racial group.

This is why the concept of race often changes, across cultures and times.

For example, when Dubois was alive, Irish and Italian Americans weren't considered 'white,' either.

But today, try telling some Boston Southie guy or an Italian grandma from Pittsburgh that they're not white.

Did something change about Irish and Italian Americans biologically?

Of course not. It's how society saw them that changed.

And it's that last bit – what race a person is seen as, and how they're treated as a result –

that ends up being a huge determinant of a person's social outcomes.

Dubois began to consider his race as a part of his identity, when he moved to the South to go to college, and then spent several years in Europe.

He saw how differently black people were treated in different places, and was disillusioned about how Americans treated him based on his skin color.

He can describe this disillusionment much better than I can:

"One ever feels his twoness," he wrote, "an American, a Negro; two souls, two thoughts, two unreconciled strivings; two warring id."

This quote reveals a really critical underlying thread in much of Dubois' work – the idea of double-consciousness.

Dubois argued that there are two competing identities as a Black American – seeing one's self as an American and seeing one's self as a Black person while living in white-centric America.

Living as a member of a non-dominant race, he said, creates a fracture in your sense of identity within that society.

These feelings are what fueled Dubois' work, which focused on the disparities and conflicts between people of different races – what we now call race-conflict theory.

Today, questions of race and identity are studied by sociologists who work on racial identity theory, which looks at how individuals come to identify as a certain race.

Dubois didn't only research racial identity, though –

he also looked at the everyday lives of black and white Americans and wrote extensively about how and why their lives differed so drastically in post-slavery America.

Let's go to the Thought Bubble to look at one of Dubois' early studies of these disparities.

In 1896, the University of Pennsylvania hired Dubois to do a survey on Black communities in Philadelphia.

His work eventually became 'The Philadelphia Negro,' the first published study of the living conditions of African Americans.

Dubois went knocking on doors, asking people questions about themselves and their families.

And there were an awful lot of doors.

All told, Dubois collected data on 9,675 African Americans.

He focused on one specific ward of Philly – the 7th ward, a historically Black neighborhood that attracted families of all classes, from doctors and teachers, to the poor and destitute.

He sat in thousands of parlors, asking questions about age, gender, education, literacy, occupations,

earnings, crime, and documented the ways in which African-Americans differed from Philly's white residents.

For example, the Black population turned out to be much younger than the White population and had a higher proportion of women.

It also had lower literacy rates, higher rates of poverty and crime, and a higher concentration of workers in the service industry than in manufacturing or trade.

Mortality rates were higher, as was the frequency of illness.

But here's what made Dubois' report especially unique:

He concluded that much of the dysfunction within Black communities came from their inferior access to things like education and more lucrative jobs.

The reason that the black population had higher rates of death and illness, he said, was because of occupational hazards, and poverty, and less access to health care.

It's hard to express just how radical Dubois' conclusions were at the time.

The problems in black communities were not due to racial inferiority, Dubois argued, but to racial prejudice.

And that was completely different from how many Americans thought at the time.

So, race doesn't exist in a vacuum.

It doesn't just imbue you with certain essential qualities.

Instead, race matters because of the power that society gives it.

For another example, let's stick with Philly and use the labor unions there in the 1890s.

Because of prejudice against Black workers, and beliefs about their abilities and morals,

trade labor unions didn't allow Black workers to join.

And because they couldn't join unions, many Black workers couldn't get manufacturing or trade work – which paid much better than service work.

And because they couldn't get these jobs, Black communities had more men out of work,

higher rates of poverty, and more criminal behavior.

Which then allowed the white workers and unions to justify their decision to not allow black workers into their union.

The prevailing beliefs about race and racism ultimately reinforced themselves.

This is what's now known as racial formation theory, a theory formalized by modern sociologists Michael Omi and Howard Winant.

Racial formation refers to the process through which social, political, and economic forces influence how a society defines racial categories –

and how those racial categories in turn end up shaping those forces.

Omi and Winant argue that the concept of race came about as a tool to justify and maintain the economic and political power held by those of European descent.

Another modern look at these issues can be seen in the work of sociologist William Julius Wilson.

He explores why Black and White Americans tend to have such different outcomes in terms of income, education, and more.

And he argues that class, not race, is the determining factor for many Black Americans.

But the reasons that these class gaps exist to begin with, come from the structural disadvantages that date back to Dubois' time.

Dubois continued to research the ways in which prejudice, segregation, and lack of access to education and jobs were holding back African Americans.

A strong advocate of education and of challenging Jim Crow laws, he clashed with another leading black intellectual of the time, Booker T. Washington, who advocated compromise with the predominantly white political system.

Over time, DuBois grew frustrated with the limits of scholarship in affecting change,

so he turned to direct activism and political writing.

In 1909, he co-founded the National Association for the Advancement of Colored People or the NAACP, and was the editor and intellectual driving force behind its magazine, The Crisis.

The NAACP fought against lynching, segregation of schools, voting disenfranchisement, and much more.

It used journalism as one of its most powerful tools, publishing the records of thousands of lynchings over a thirty year period.

And it used lawsuits, targeting voter disenfranchisement and school segregation in decade-long court battles.

And, after DuBois' time, it went on to become part of many of the landmark moments in the fight for civil rights,

including the Brown vs. Board of Education case, the Montgomery Bus Boycott and the March on Washington.

Modern sociologists continue Dubois' work on racial politics, asking the question:

How is race intertwined with political power, and the institutional structures within a society?

Sociologist Eduardo Bonilla-Silva, for example, argues that we now have what he calls "racism without racists."

What he means is: Explicitly racist views have become less socially acceptable, so fewer people are willing to say that they don't think Black and White Americans should have equal rights.

But, as Bonilla-Silva points out, that doesn't mean racism is a thing of the past.

Instead, he says, structural racism – the kind that's entrenched in political and legal structures – still holds back the progress of racial minorities.

Take, for example, the fact that the median wealth of white Americans is 13 times higher than the median wealth of black Americans.

Now, you could look at that and say, well, black people just aren't as good at saving as white people.

After all, it's not like there's anything legally preventing them from making or saving more money.

But that completely ignores the ways in which wealth builds up over generations.

Past generations of Black Americans were unable to build wealth, because they had far less access to higher incomes, banking services, and housing.

These ideas about how the structures of power interact with race may have their origins in Dubois' work, but they continue today.

And so do his studies of racial resistance.

Researchers of racial resistance ask: How do different racial groups challenge and change the structures of power?

Sometimes racial resistance is easy to see in society.

Think the Civil Rights movement of the 1950s and 60s, or Black Lives Matter today.

But sociologists can also look at more subtle forms of resistance, too, like resistance against racial ideas and stereotypes.

For example, sociologist Patricia Hill Collins has written about the different relationships that black and white women have had with marriage and staying home to raise a family.

In the feminist movement of the 1960s and 70s, one of its key issues was the exclusion of women from the workforce.

Entering the workforce was seen as a form of resistance.

But Black women have, for most of American history, been forced to work, or needed to work to help support their families.

For them, Collins argues, joining the workforce is not resistance.

Instead, staying at home to care for their families can be an act of resistance against society's expectations for Black women.

All of these modern fields of study within race-conflict theory – racial identity,

racial formation, racial politics, and racial resistance – they all have their origins in the work of one sociologist: W.E.B. Dubois.

Today we talked about W.E.B. Dubois, one of the founders of sociological thought and the founder of race-conflict theory.

We talked about race and how our understanding of how we define race has changed over time.

We talked about Dubois' idea of 'double-consciousness' and how it relates to the modern day field of racial identity.

We introduced the idea of racial formation and used Dubois' survey of African Americans in Philadelphia to look at how economic, political, and social structures affect how we perceive different races – and vice versa.

And finally, we looked at the activist side of Dubois' life as co-founder of the NAACP and editor of the Crisis, and discussed how modern day sociologists study racial politics and racial resistance.

Next time, we'll take a look at some of the sociologists who were at the forefront of a different type of conflict theory: gender-conflict theory.

The occasion is used to parade North Korea's highly-synchronized soldiers and latest weaponry,

— It's been an eventful weekend in Pyongyang.

At 4 a.m. on Saturday, we were loaded into a van

and driven to Kim Il-sung Square, in the heart of the city.

— Choe Ryong Hae, one of Kim Jong-un's closest confidantes, addressed the crowd.

Kim Jong-un looked on approvingly.

— The march took place at one of the tensest moments in years between the U.S. and North Korea.

The U.S. Navy's Carl Vinson Strike Group was approaching the peninsula,

and satellite photos had suggested an imminent North Korean nuclear test.

— The following morning, thanks to emails from the outside world,

we found out the nuclear test hadn't happened.

But while we were at the parade,

the North Korean military were preparing a medium-range missile test

off the country's eastern coast.

According to the U.S. military,

the missile "fizzled out" shortly after launch.

Some analysts are claiming it could have been sabotaged by U.S. cyber warfare.

We continued our strictly-controlled press trip,

and were politely asked to avoid the topic of the failed launch.

A Skateboard with WINGS? Worst Skateboard Inventions of All Time. - Duration: 11:28.

Hey guys, Rad Rat is back with some of the worst skateboard inventions of all time.

Skateboarding deserves to be documented, but some of this stuff is better left in the past.

Let's get started.

Welcome to Rad Rat Video.

Here, we learn something new about skateboarding 3 times a week, on Mondays Wednesdays and

Fridays.

Stuff like trick histories, game reviews, and learning tricks in the Shred School.

In my research, I've come across a lot of really weird products that have been made

for skateboarders over the years.

For the sake of this video, these gimmicks are random skate products that don't really

do anything useful, or were at least way overblown.

A good example is my first one, the Skate wing.

Just look at this thing!

Can you imagine carrying this thing around?

You'd have to carry it in the back of your truck and dump it out in the street.

The Skate wing is an Australian invention.

Apparently they made them out of wood, but later made plastic too.

Probably because the price was too high.

They're apparently also known for getting a lot of stress cracks because of the weird

shape.

It was designed by a yacht racer.

For some reason.

It's 33 inches long and 30 inches wide.

It's got skids and even RAILS on it, as if you'll be doing boards slides on this thing!

Can you believe that?

So it's got handles and extra wheels on the sides.

I guess you can crouch down and carve so hard that you tip over.

It might be fun for about 10 minutes or so.

That's not the only weird specialty board either.

How about this one?

The Wheelee board.

It's got this super long kicktail with a third truck on it.

And then, of course, a little third anti-kicktail at the end of that.

So what's the point?

Spinning 360s, supposedly.

They already make special steel wheels for spinning 360s.

But at least you can swap those out when you're done.

This thing you have to live with looking like that.

Oh, and you can do 'power turns', whatever that means.

But it comes textured, so you don't need grip tape, which is nice.

Next up is bridge bolts.

They clip over your deck and take the place of 2 bolts each.

I have no idea why.

It saves you, what?

1 second each when you set up a board.

Apparently some people set them up sideways so they'd catch more on ollies and give you

more air, but that doesn't seem to be their purpose.

I couldn't tell exactly what they were for.

It seems like it would do nothing but get in the way.

By the way, notice the doubled holes on the nose here.

That's an adjustable wheelbase.

I wouldn't call that a gimmick because it was helpful for making the board adaptable

for street and vert.

You could choose one set of holes or the other.

Next is lappers, which were actually really popular.

You might have heard about them before and wondered what they were.

Here's a picture.

They're these really weird plastic sliders that cover up the kingpin and the rest of

the hanger.

The idea is, on lipslides or disasters, you're less likely to hang up because they'll ease

you back into the transition.

I'm sure it would help every now and then, but they encourage bad style where you can

just kind of flop in and put, and they add a bit of needless weight to your setup.

I saw an interview with a pro from back then, I don't remember who, and when he stopped

using them, he said he never got why he started in the first place.

They were pointless.

I have two more weird truck add-ons for you.

First is copers.

They're just plastic covers for the grinding surface of your trucks.

A lot of this stuff would have helped make your board last longer, but man.

One of the most satisfying things about skating is grinding stuff.

Forcing your board to grind along the concrete or whatever.

And it sounds awesome too.

But copers ruin all of that.

Plus, again, they add weight.

So what's the point?

The other one is z rollers.

Kind of the same thing, but built in, and metal.

They had these rollers that would actually roll along the coping.

Again, these must have felt horrible, like screeching rusty grinds.

At the same time these weird things were popular, Grind King invented inverted kingpins, and

guess what?

They didn't catch on for a long time.

That's just the start of the protective gear though.

Of course there are tail skids, nose guards and rails, which I get.

The nose guard is weird, but I don't think it's too crazy.

But did you know there was a tail skid for the TOP of the tail?

They were called heel bones by Powell Peralta.

The idea was that they would help with your precision?

So you get the steeper tail without the unwanted benefit of actually getting any pop out of

it.

But I don't think you would grip over it, so it would actually hurt your skating more

than it could possibly help it.

Also, we've got lace savers.

These actually made sense to a degree, but obviously didn't stick around for very long.

These weren't too terrible of an idea, but shoelaces are pretty cheap.

They remind me of this shoe cover product I've talked about before in my new technologies

video.

But I think in any case, adding something extra to your shoes and giving you less board

feel is generally a bad thing, you know?

But they didn't just try to protect your board and shoes, you could also protect your

fingers.

Those awesome 80s fingerless gloves would leave your poor little fingies in danger,

so Santa Cruz invented Rip Grip.

These are stickers that are thick and spongy and you would put them in places that you

grabbed the board for some extra cushion and grip.

These were hugely popular back then in the late 80s - and they came in all kinds of crazy

bright 80s designs.

It's kind of crazy - a decent complete back then would have run you almost a hundred bucks

- not too far off from today.

But once you buy a few packs of Rip Grip, tail bones, nose bones, rib bones, and heel

bones from Powell, lappers and copers, bridge bolts… you could have almost doubled that

cost.

But to be fair - that board would have lasted forever.

I see boards like this at antique shops from time to time.

If I ever see a good one, I want to buy it and try to skate it.

Usually they're either painted over, or they're off brand Walmart stuff from back

then.

And they want 60 bucks for them.

BUT - they're still standing.

These boards could take a serious beating.

OK, I've got a few more left for you.

First, single-bearing wheels.

These came out in the early 90s.

On paper, they kind of seem like a good idea.

Use half the bearings - right?

But in practice, they didn't work all that well.

Apparently, the bearing would pop pretty easily.

Which makes sense.

There's just a spacer in there, which doesn't really seem to lock in all that well.

Plus, the single bearing is taking the stress and force of both bearings together.

So think about doing a bluntslide.

You ollie in, and the wheels stop.

Then your axle nut on this side and the truck on this side slam into them and force them

to slide forward.

That's a lot of force for a single bearing with some spacers in it.

It's no surprise they didn't work out.

Next is skyhooks!

I'm sure everyone has tried something like this when they first started skating.

These are devices that attach to your nose and tail and lock your feet in.

Then you can jump and the board stays with you.

It's fun to do by tying your shoelaces to the board or duct taping your shoes down.

But this was an actual product that you would buy and use permanently.

It's really weird.

It seems like they came out in the 80s, even though both vert and flatground ollies were

already invented.

I mean, this picture looks like it's from the 70s, so I guess I'm not entirely sure

about that, but they lasted into the 80s at least.

Weirdly, these have caught on with longboarding a bit more these days.

You can find a lot of people using them.

Maybe they help you stay locked in when you're rolling over rough surfaces or something.

Or you can powerslide with them without slipping off.

I'm not entirely sure.

Lastly, and worst-ly, is these.

So, there are scooters, which are bad enough as it is.

But at least they're a thing.

You can turn the handlebars and carve around on it I guess.

But this is just a way to attach handlebars to a regular skateboard.

You can't turn with them, and they do absolutely nothing except make it impossible to do any

tricks at all.

I'll never understand these.

Is skateboarding really so hard that you need to lean on something while you're riding

around?

And the worst part?

You would see ads for these in legit skateboard magazines back in the day.

It's just embarrassing.

Believe it or not, these still exist.

There's a brand called CaliBars that is still making skateboard handlebars.

Supposedly they're for helping kids learn how to skate, and they kind of take the high

ground over scooters, saying that it's at least a skateboard.

As long as it's 4-year-olds, it might not be the worst thing, but come on.

So those are the ones I found for this time.

If there�s a will, there�s a way Congress and Trump can still fix health care law

Now that the GOP effort to repeal and replace the Affordable Care Act is in limbo, is there

a way to make it work better?

Democrats and Republicans don�t agree on much when it comes to the controversial federal

health law, but some party leaders from each side of the aisle agree it needs repairs.

�No one ever said the Affordable Care Act was perfect,� said Senate Minority Leader

Chuck Schumer, D-N.Y., on the Senate floor March 27. �We have ideas to improve it.

Hopefully our colleagues on the Republican side do as well.�

A day later, Speaker Paul Ryan, R-Wis., said, �We all want a system in health care where

everybody can have access to affordable coverage, where we have more choice and competition.�

And several GOP senators have moved away from the party�s long-held call for a total repeal

and are offering bills that would amend the measure.

Health policy analysts say that some of the health law�s marketplace problems could

be improved with a bipartisan spirit. Here are some of the possibilities:

Stabilize The Insurance Market

Insurance companies have only a matter of weeks before they must tell the federal government

and/or individual states whether they plan to offer coverage in 2018 on the health law�s

online marketplaces, which serve customers who don�t get job-based or government insurance.

As of now, many companies say the uncertainty of what the market will look like � or the

rules under which they will operate � is making that decision difficult. At least one

insurer, Humana, has already said it would not offer coverage.

Two key moves that insurers are looking for from the administration are a promise to continue

providing certain subsidies for those with lower incomes and enforcing the requirement

for most people to either have insurance or pay a tax penalty.

The subsidies � known as �cost-sharing reductions� � are different than the tax-credit

subsidies that many marketplace customers get to help pay their premiums. The cost-sharing

subsidies help those with incomes between the poverty line ($20,420 for a family of

three) and nearly 2� times that ($50,400 for that same family) pay their deductibles

and other out-of-pocket costs. The House of Representatives sued the Obama administration

in 2014 for providing the subsidies without a formal congressional appropriation for the

money, and a federal judge sided with the lawmakers.

The Obama administration appealed the decision, but if the Trump administration were to drop

that appeal, the subsidies would disappear. At a House hearing March 29, Health and Human

Services Secretary Tom Price would not say what the administration plans to do about

the lawsuit or the subsidies.

�I�m a party to that lawsuit and I�m not able to comment,� he said. But Ryan,

who is also a party to the suit, said March 30 that he believes the administration should

continue paying the subsidies until the lawsuit is resolved.

The administration has been similarly quiet about how strictly it will enforce the �individual

mandate� that requires most people to have health insurance or pay a fine. On his first

day in office, President Donald Trump issued an executive order directing federal agencies

to �minimize the unwarranted economic and regulatory burdens� of the health law. But

other than the IRS backtracking on a plan to enforce the mandate more strongly, little

has happened on that front.

Yet those two provisions together � the cost-sharing subsidies and the individual

mandate � could result in 25 to 30 percent increases in premiums if they were to disappear,

said Andy Slavitt, who oversaw the health law for the final years of the Obama administration.

Assuring that the subsidies will remain and the mandate will be enforced �would send

a strong signal to (insurance companies) that they should continue to participate in the

market,� he added.

Some GOP health policy analysts � including economist Gail Wilensky, who previously ran

the Medicare and Medicaid programs � have proposed replacing the individual mandate

with penalties for signing up for insurance late, which is what Medicare does. Republicans

did that in their proposed replacement bill, by adding a 30 percent premium surcharge to

those with a break in coverage longer than two months. But insurance actuaries and the

Congressional Budget Office said that might eventually prompt fewer people to enroll because

it would encourage healthy people to remain uncovered.

Entice People To Enroll

Getting young and healthy people to sign up for coverage is not just a benefit for them.

If there are not enough healthy people in the insurance pool, then premiums rise for

everyone, because risk is being spread across a mostly sicker population. And someday even

the healthy people will need medical care.

But it takes more than just the requirement for coverage to get people to enroll. Slavitt

said it requires a real effort by both federal and state officials to reach people and help

them understand that having health insurance is a good thing, even if they are healthy.

�What they should be doing is increasing marketing and the outreach budget,� he said.

�You�re trying to reach people who are uninsured and are unsure how it all works,

and it does take a lot of hand-holding.�

So far, however, the administration�s only move on that front was to cancel ads encouraging

people to sign up at the end of the enrollment period that overlapped with Trump�s first

days in office. The HHS Inspector General is now investigating the results of this action.

Some analysts have estimated canceling the last-minute push lowered enrollment in the

exchanges by as much as a half-million people.

Help Offset Insurer Losses

Democratic lawmakers who wrote the Affordable Care Act knew that the market might be hard

for insurers to navigate for the first few years, and they built in several programs

to help reimburse those who lost money.

Republicans, however, blocked funding for one of the major programs, which was intended

to reimburse insurance plans that enrolled sicker-than-average populations for the first

three years of the marketplace operations. Republicans called the money �insurer bailouts.�

The loss of that money was a major reason for the collapse of many of the nonprofit

insurance co-ops created under the law and some other insurance companies said it contributed

to their decisions to leave the marketplaces.

Now, however, there are indications that Republicans might support some efforts to provide more

funding for insurers.

On March 13, Price sent a letter to governors encouraging them to apply for waivers of the

ACA rules in order to make insurance more affordable and available in their states.

Among the state innovations singled out in the letter is a �reinsurance� program

in Alaskathat helps insurers pay for extremely high-cost patients. That plan, said Price,

�significantly� offset what was a projected 40 percent premium increase in the state,

and might be an option elsewhere under the waivers, which could allow states to get federal

funding for such a program.

And the GOP health bill, the American Health Care Act, included $100 billion for a �Patient

and State Stability Fund� that states with limited insurer competition could use to lower

costs and help encourage insurers to stay in the market.

�They have a potential fix staring them in the face,� said Larry Levitt of the Kaiser

Family Foundation of the GOP proposal for a stability fund. �It was a clever mechanism

because the states could use it for any of a variety of purposes.� (Kaiser Health News

is an editorially independent project of the Foundation.)

Assist Patients With High Out-Of-Pocket Costs

Democrats and Republicans agree that people who buy their own insurance are paying too

much out-of-pocket, in premiums as well as deductibles and cost sharing.

Democrats mostly want to increase federal subsidies to help with affordability � something

Republicans are not likely to embrace.

But there are other ways to lower consumer spending.

For example, Sabrina Corlette of Georgetown University�s Center on Health Insurance

Reforms calls for �smarter, not skimpier benefit design.� One way to do that is to

set federal rules to push insurers that offer coverage with high deductibles to add more

benefits that would be available without paying thousands of dollars first, like a few trips

to the doctor or urgent care center or a few prescriptions. She writes that could keep

people from dropping coverage because they feel they are not getting any value for their

premiums. And if those mostly healthy people feel they are getting benefits they might

use, they are more likely to continue to purchase coverage, thus reducing premiums for everyone.

Another potential way to lower insurance costs is to lower health care costs. Even if there

are multiple competing insurers in an area, if there�s one dominant hospital system,

it can pretty much charge whatever it wants.

�There�s no other price in the U.S. economy that�s growing as fast as a hospital price,�

said Bob Kocher, a former Obama administration health official now at the venture capital

firm Venrock. And in areas with not much hospital competition, �prices are 30 to 50 percent

higher for everything.�

But how to get hospital prices down is almost as hard as regulating insurance. Kocher said

one way would be for federal regulators to be more discriminating about approving hospital

mergers, which tend to give hospitals more negotiating power over insurers.

More controversial would be to require hospitals that dominate their markets �to just accept

Medicare prices� from marketplace insurers, Kocher suggested. While that would tend to

bring prices down, it�s not likely to fly with free-market Republicans.

Every year, millions of people are infected with diseases you've probably never heard

of.

There's tons of media coverage about things like malaria, Zika, and HIV — all of which

pose serious threats to public health.

But there are many more diseases out there that are just as deadly -- yet not as infamous.

They're called neglected tropical diseases, or NTDs, because historically they haven't

been paid as much attention as other diseases, and they mainly affect people in tropical

areas.

But these days, global healthcare initiatives are paying attention to them -- working to

find ways to prevent and treat these diseases.

The deadliest NTD — caused by the second-deadliest parasite in the world, after malaria's — is

schistosomiasis, also known as snail fever.

It's caused by a parasitic worm, called schistosoma, that's transmitted through

water that's been contaminated with the parasite's eggs.

The parasites use certain kinds of snails in the water as hosts, to grow and multiply.

Eventually, the parasites' larvae are released into the water, and that's when anyone who

comes in contact with the water is at risk, because the larvae can burrow through the

skin.

Infected people often don't have symptoms for the first month or two, though they can

sometimes end up with a rash or other symptoms like fever, aches, and a cough.

Symptoms are much more common later on, once the larvae have grown into adults and laid

eggs.

That's when the infection is considered chronic.

But the symptoms of chronic schistosomiasis aren't caused by the full-grown worms — the

worms can actually incorporate proteins from the host to trick the immune system into ignoring

them.

It's the eggs that cause an incredibly strong immune response, which can strike almost anywhere:

the intestines, the lungs, the bloodstream, even the brain.

So schistosomiasis can cause all kinds of symptoms, from diarrhea to wheezing to seizures.

More than 200 million people have schistosomiasis right now, and about 200,000 people die from

it every year.

The main way to treat it is to prevent it: if people are regularly given a drug called

praziquantel, even if they don't have the disease, they're less likely to develop

a severe infection later on.

But the best way to protect people in the long term is to make sure communities have

access to sanitation and safe drinking water, to stop the parasite from spreading.

Another neglected tropical disease is Chagas disease.

It's most prevalent in Central and South America, where about 8 million people have

the infection, and about 10,000 die from it every year.

Chagas is caused by a parasite called Trypanosoma cruzi, a single-celled protozoan.

The parasite spreads through the feces of infected insects, often known as kissing bugs.

Usually, the parasite enters the body through mucous membranes — like in the eyes and

mouth — or through broken skin, when someone scratches a kissing bug bite.

The best time to treat the disease is just a few days after infection, but most people

don't show any symptoms for weeks.

And even in those who do, the symptoms are pretty mild and nonspecific — like fever,

a rash, and diarrhea.

And then something frightening happens: After this initial phase, Chagas infection usually

goes dormant -- for years.

After as much as a decade or two, about 30% of infected people then develop chronic, life-threatening

symptoms — including swelling of the heart, or the intestinal tract.

So, prevention and early detection are very important here.

But as with other NTDs, it often affects people who don't have easy access to healthcare.

Fortunately, efforts are underway to help those at risk, mainly with the help of insecticides.

If you kill the bugs, they can't spread the disease.

Then there's African trypanosomiasis, otherwise known as African sleeping sickness, which

affects parts of sub-Saharan Africa.

Like Chagas, this disease is caused by parasites carried by insects — in this case, by the

bite of the tsetse fly.

In the first phase of sleeping sickness, the parasite reaches the peripheral nervous system

— the nerves that aren't part of the brain or spinal cord.

Here, symptoms include things like fever, joint pain, and itching, which can be caused

by lots of different things.

So — again like Chagas — sleeping sickness is hard to diagnose at first.

But symptoms become more severe when the parasite invades the central nervous system — the

brain and spinal cord.

Then, it can cause convulsions, confusion, and the symptom that gives the disease its

name: uncontrollable sleepiness.

If left untreated, sleeping sickness can be fatal.

But it is treatable with combined medications, even in the second stage.

And initiatives to treat sleeping sickness have become more and more successful in recent

decades.

In 1995, there were an estimated 300,000 new cases of African sleeping sickness.

But since then, health organizations have been working on testing people for the disease

and getting treatment to those who need it.

They've also been using insecticides to kill tsetse flies.

By 2014, there were fewer than 15,000 new cases of sleeping sickness.

There are lots of other NTDs out there as well.

But they aren't being neglected anymore, either, and that's making a big difference.

you can see we do a lot of air

conditioning work we got a couple of

machines here a couple old ones over

there but right now I want to talk about

hybrid now if you own a hybrid it is

important that you know that takes

special equipment to do hybrid air

conditioning so if you call a shop and

you ask them I've ever machine for

hybrids and they say no run and let me

tell you why because the peg oil

conducts electricity and regular oil for

the hybrids that's not as a matter o

fact Toyota 's got its own oil Honda's

got its own oil Ford it's got its own

oil and that and you cannot in the mix

them because you will do damage to the

air conditioning system and if you

prepare oil you get the car would even

shut down on you so it is very important

that that you know that hybrid takes a

special machine and if you come over I

mean we can wait on a hybrid I've only

got one machine but it's not as busy as

the other one we have a comfortable

waiting room it takes two hours to do

the job and the reason it takes two

hours is I put it on vacuum for one

hour to remove the moisture very

important people don't even know that

why do you put it on vacuum

asked the mechanic if he hasn't watch

this video he doesn't know why so you

remove the vacuum vacuum combined with

refrigerant creates a chemical that's

really really bad for the system so

again you can wait for it a couple hours

I can run you down to the local library

to a local restaurant would give you a

ride home and pick you up you want to go

home for a couple hours drop you off and

we pick you up you got multiple drivers

or me and my partner can drop you pick

you up and drop you off and if by any

chance you need a part and we need to

keep a cover knife or a repair we

provide a rental car at my expense for

one day and we hand washed the because

we give a three year warranty on our

work

unheard of in this industry and with all

that combined why would you go anywhere

else just remember air-conditioned is

one of the comfort features and if

you're on a hybrid it takes a special

machine don't just let anybody work on

it and they have to know your condition

not just a matter of adding refrigerant

condition is shocked that I'm told

smaller than mine I it's about 10-15

compressors a week a week when the

summer time I maybe replace one two

maybe and that's because they did not

get charged correctly again you want it

done correctly you can come to resist on

How to Charge Auto Air Conditioning Huntington Beach - Duration: 7:53.

automotive you watching this video is

because you need help in recharging

air conditioning system let me show you

my shop but first let me show you my

landscape I love my landscape I'm here

12 hours a day this give me a lot of

peace we're in the process of recharging

two different vehicles ok so I'm going

to show you how to start from the

beginning the first thing you do if

you're doing it at home it's going to be

a little bit harder for you because what

are you going to do with the refrigerant

you're going to have to if you come to a

shop like mine I can identify it recover

it if you one of my regular customers

I'll help you out I charge a minimum or

nothing or you can vent it into the

atmosphere which I don't recommend but

here when recharging took because this

one was low right here coming in let me

show you this one was low this one came

in had two pounds of refrigerant and it

takes 3.5 this one was 1.5 low on

refrigerant the first thing we do is we

remove we recover the refrigerant we

check the pressures which already can do

at home we check the pressures okay and

then I recover the refrigerant and then

using a tool like this you come in and

you remove this Schrader valves they

just unscrew you pull it out and then

you put the new one in and then you want

to replace them because they'll eat a

lot and then just kind of screwing it

like the Schrader Valve on a car and

then you screw it in and then

this nice and tight and then you go to

the other side and then this is your low

side and this is your high side

okay now on your high side same thing

you're going to remove this Schrader Valve

some of them you have to replace the

whole piece but if you get any questions

give me a call you take the new

Schrader Valve you put it in and then

you just screw it in see that once you

screw it in nice and tight then you're

ready to go to the next step then and

then do it quickly because if you don't

have a machine to put it on back home

you let moisture in so when you remove

it put it in quickly do not ever leave a

system open because it gains moisture

inside the accumulator there's a

silicone bag that removes the moisture

so you don't want to soak it now on this

one over here we're already charging it

now this one when he came in he gave me

a 25 pounds on the low side 125 on the

high side and the captain chill was 45

degrees we took got 2.1 pounds we

replace the straight about same

procedure replacing Schrader belt and

put it on back in for an hour to remove

the moisture now we charged it up once i

charged it up then my pressures got

better are you got about 35 + 150 and

you kept them to show us 40 degrees a

couple of things that are very important

if you're doing at home you got those

little cans of gauges that worthless and

because pressure can change even if you

put the right amount in pressure can

change between a cold a warm day

a hot day your pressures are going to change so

don't go by there as little as ten percent

under charge can send your compressor to

an early grave so you don't want an

undercharge or overcharge if you

overcharge you develop a head pressure

so you don't want to do it you

definitely do not want to do any of that

another thing you can do is if you have

if you have the you kind of do a visual

leak take a visual leak

check your condenser check your up check

your hoses if you are if you got the

hoses check the hoses like for example

that holds right there you see how it's

nice and dry okay know if you see only

then then there's a sign that might have

a leak so kind of do a visual and all

your hoses on the condenser if you see

an oily spot on the condenser coil then

you probably have a leak take it or if

you come by and I and you show it to me

i can tell you whether I think you have

a leak or not because when you lose

refrigerant you lose a little bit of oil

now when I do a recharge about every

third recharge I add an ounce of oil so

figure out a way to add an ounce of oil

because if you keep losing the prison it

doesn't take a lot of oil get some sense

the money takes six seven ounces so if

you lose one or two hey you're losing

more than ten percent twenty percent so

make sure you have all that into

consideration again you're going to get

dumped the refrigerant and the good part

of us started with an empty system if

you're doing at home is you know how

much to put in because if you put in too

much or not enough it is no good all the

touching undercharging can send the

compressor to an early grave and on some

cars like if you own a Cadillac for

instance and some of the Volkswagens if

you own a Cadillac and the air

conditioning system stops working

because it's low then you can think you

cannot get a restart unless you reset

the computer so you got to get somebody

to that don't go crazy think you've got

an electrical problem you got to make

sure that it gets reset and when this

system was low the the gauge is he right

now or gauges again a little bit high

because I got the condenser covered up

front you got no air flow so but it's

still good

now when you do it when you do a

conditioning part of its electrical

don't take it for granted that that your

problem is the refrigerant succeed the

compressor works and also you got to

have the proper equipment now if you

have the properly we have an identifier

for air conditioning refrigerant we have

a scale while I have it I don't know

because I've always had it and I haven't

gotten rid of a disc very accurate

skills if you go to the older machines

that I still have next your work I do

not trust the scales anymore that's like

I knew us if you own a hybrid even you

own the hybrid don't mess with it you

think it takes a special machine for

hybrids because you can mix the oils you

can cause hvac in the air

conditioning system you got any

people don't know that to do hybrid

hybrid just left I just did an A/C service

on it you need a special machine and you

need special equipment and as you can see

i have two machines over here for

regular cars they're fairly new and then

I got one just for a hybrid this one

only comes out when hybrids comes

in because we do quite a bit of hybrid

and I bought this machine about 2-3 years

ago in order because I specialize in

electrical and I could not do hybrids

maybe a little bit longer than that so

and here's the thing you cannot use

regular peg oil on a hybrid car because

it convinces electricity something with

the electricity and it can send havoc to

the vehicle in many different ways not

only to the compressions to the AC

system but on something you're a hybrid

it will literally shut the car down

because of the conduction of electricity

so when you do work on a hybrid is very

very important that that you know that

it takes a special machine if people

don't know and you cannot just add freon

into a car anymore or refrigerant is the

proper word freon is the name of a

company like Ford but with that said

when you have a hybrid make sure to use

a hybrid machine make sure they they put

in the proper amount a lot of the

vehicles nowadays take one pound so if

you're two ounces low you're more than

ten percent under charge ten percent on

the charge can send you compress to an

early grave now you know around here in

Huntington Beach we get our hot days and

we love her comfort in Huntington Beach

we get our hot days and as soon as first

keep your A/C serviced correctly

you will probably never have to buy a

compressor unless the manufacturer had a

problem with those compressors ten

percent under charge can send your

compressor to an early grave I bet you a

lot of you shop owners don't know why if
