Youtube daily report Aug 21 2017

I think it's - I'm a believer in the precautionary principle as I've just said, and I think

we have to worry about possible consequences of things that we do, and the ability to edit

our own genomes is one thing we ought to worry about.

I'm not sure it's so much an ethical problem as a more practical problem.

What would the consequences be?

Would the consequences be bad?

And they might be.

I think it's worth noticing that long before CRISPR long before it became capable of editing

our genomes in anyway we have been editing the genomes of domestic animals and plants

by artificial selection, not artificial mutation, which is what we're now talking about, but

artificial selection.

When you think that a Pekingese is a wolf, a modified wolf, a genetically modified wolf—modified

not by directly manipulating genes but by choosing for breeding individuals who have

certain characteristics, for example, a small stubbed nose, et cetera, and making a wolf

turn into a Pekingese.

And we've been doing that very successfully with domestic animals like dogs, cows, domestic

plants like maize for a long time, we've never done that to humans or hardly at all.

Hitler tried it but it's never really been properly done with humans I'm glad to say.

So if we've never done that with humans with the easy way, which is artificial selection,

it's not obvious why we would suddenly start doing it the difficult way, which is by direct

genetic manipulation.

There doesn't seem to be any great eagerness to do it over the last few centuries anyway.

A lot of people have problems with what they call designer babies.

You could imagine a future scenario in which people go to a doctor and say, "Doctor, we

want our baby to be a musical genius.

Please edit the genes so that we have the same genes as the Bach family had or something

like that to make them into a musical genius."

I mean that horrifies many people.

It's got totally obvious why that's anymore horrifying than parents who are ambitious

for the musical future of their child forcing the child to practice the piano three hours

a day.

There are differences, of course, forcing it to practice is maybe unpleasant for the

child but it doesn't go into the next generation, changing the genes does so there is a difference

there.

But at least people who shudder with horror at a designer baby who's a musical genius,

people who shudderat horror at that, why don't you shudder with horror at forcing a child

to have music lessons when it doesn't want to or practice music when it doesn't want

to?

I think although there is an analogy between technological evolution and biological evolution,

it's dangerous to push that analogy too far.

I've just been decrying the view that everybody is an expert.

And I'm not an expert in technology and I think that every scientist needs to admit

when they don't know.

The analogy is there, things change gradually, the evolution of the airplane starting from

the Wright brothers in the beginning of the 20th century until the present, it's been

spectacularly fast but it is gradual evolution and it looks, to some extent, like biological

evolution.

But whether it occurs by natural selection that's open to argument.

You could say that it does occur by a form of natural selection, but it's not the same

kind of natural selection.

So I don't regard myself as a biological evolutionist as qualified to talk about technological

evolution.

I'm interested in the analogy but I think it only is an analogy and it doesn't go

too far.

#When is the Champions League draw, where will English teams be seeded and who could they play? - Duration: 5:18.

When is the Champions League draw, where will English teams be seeded and who could they play?

FIVE Premier League teams will compete for Europes most prestigious club prize this season. Manchester Uniteds victory over Ajax in the Europa League final ensured they will be in the hat for the Champions League.

Five Premier League teams could be playing in the group stages of the Champions League next season. But the route to the final in Kiev could prove longer for some clubs than others.

Heres the lowdown on the draw dates for the Champions League and which sides the English clubs could face.

Liverpool take a 2-1 lead over Hoffenheim in their play-off as they aim to get in next Fridays group stage draw. The second leg is on Wednesday, August 23.

Liverpool will play in the Play-off round of the Champions League. Here are the draw dates for this seasons Champions League….

The draw for the group stage will be in Monaco on Thursday, August 24 at 5pm BST. You can follow all the news from the Principality on SunSports live blog.

Quarter-finals . Semi-finals.

English clubs seeds for the Champions League draw. Pot One contains Champions League holders and the champions of the top seven leagues across Europe.

Therefore Chelsea are guaranteed be in pot one for the draw. Pots 2, 3 and 4 contain the remaining teams, seeded on their club coefficient ratings.

Manchester City and Manchester United are guaranteed to be in pot 2. Tottenham are in pot 3, as their co-efficient ranking is lower than Europa League winners United.

Chelsea will play in pot one of the Champions League group stage draws.

Who could Chelsea play in the Champions League?. The Blues could face Barcelona, Atletico Madrid, Paris Saint-Germain, Borussia Dortmund or Porto. Roma, RB Leipzig, Basel, Feyenoord and Anderlecht are also possibilities. Chelsea could meet Barcelona in the Champions League group stages.

Who could Manchester United play in the Champions League?. Jose Mourinhos side could be drawn with Real Madrid, Bayern Munich, Juventus, Benfica, Shakhtar Donetsk, Monaco or Spartak Moscow.

Roma, RB Leipzig, Basel, Feyenoord and Anderlecht could also face a trip to Old Trafford. Who could Manchester City play in the Champions League?.

Like their rivals United, Man City are in pot two and face the prospect of playing the same teams. Therefore they will definitely play one of the following: Real Madrid, Bayern Munich, Juventus, Benfica, Shakhtar Donetsk, Monaco or Spartak Moscow.

They will also draw Roma, RB Leipzig, Basel, Feyenoord or Anderlecht.

Manchester City could take on the likes of Real Madrid or Bayern Munich. Who could Tottenham play in the Champions League?.

They will play one team from pot one which includes: Real Madrid, Bayern Munich, Juventus, Benfica, Shakhtar Donetsk, Monaco or Spartak Moscow. Who could Liverpool play in the Champions League?.

Liverpool need to win their Play-off fixture in order to book their place in the group stages.

A total of 20 teams play in the play-off round but because the Reds are seeded for the draw, it would mean they will play a weaker opposition. They could currently play: Steaua Bucharest, Young Boys or Nice.

Deluxe Skins, Hydroid Reworks + Arcane 3.0! - Devstream 97 Recap N Review [Warframe] - Duration: 4:40.

Ahoy thar, Legion!

Tis the Monday after a Devstream so ya know we gotta Recap N Review for ya!

Console Squad!

You got yourself an update recently, so hope yer enjoyin' that.

I believe we all on the same page, more or less, on the Hydroid rework, for better or worse, but in the end DE said

that's about enough of that and he's not gonna get any more changes now

They said he might be too powerful, which I'll let the Rant Roundup weigh in on that one to see if the players agree -_^

Of course the actual reason for this rework is due to the changin' of the guards of Prime Access!

Oberon's is getting replaced with Hydroid around August 29th.

So if you wanted Trinity Prime, Dual Kamas Prime, or the stylish Kasava Kubrow Prime collar,

farm ye their relics now, or forever rip in peace.

Tennogen round 9 was just released, but round 10 will be simultaneous on all platforms on

Halloween with spoopy looks in tow.

Speakin' of Fashion Frame: Deluxe Skins!

Ember Deluxe has a rather checkered past and is apparently getting reworked entirely,

Mag's was shown briefly and we can supposedly expect this before Halloween, but what do we have here?

Is this a new frame?

Could be based on the design.

But it's actually a deluxe skin of....

Nekros!

I have no idea as to why or how they came to this look, but there it be and hope you

enjoy it more than I.

Personally I think it looks a lot more like the Zephy deluxe skin, and Zephyr fans I know

you've been waitin' ages for yours, and DE's gave an update on it!

It was, and I quote: "The only update is that there is no update."

These pictures of it right here I have saved in my folder since,

Drumroll, please!

August 21st 2016.

Been over a year since we've seen it.

Long enough to declare it legally dead!

( I dunno if that's true, or not, but it sounds funny XD )

You turkey lovers out there have my condolences.

I don't know why do they enjoy torturing you so much. I'm sorry.

Glass Frame!

Not much new, other than her default colors and codpiece still scream FemRhino to me,

so I'm still shippin' that.

Her initial abilities must have sucked as they are reworking them,

but as long as that chest piece remains in place

that one gif I put on reddit will get some use, so there's that.

No hard yes on a quest for her, but they said it'd be Plains related?

Speakin' of Plains, loot there will be different as normally it's handed out at extraction

and here is a wide open space to explore.

While they are adding several places to extract to save you time goin' all the way to the

main entrance, completing mini-missions and objectives will sync your loot with the database,

so ya won't miss that fish ya just fished.

We'll also have mining or Prospecting to do on rocks or caves to get Gems we can add to

the melee crafting system which is also a rework of the Arcane system as well.

Raid system stays the same, this is a new and more attainable version.

Get gems from rocks, put gems in weapons, there ya go.

No details yet, but I suspect it has something to do with the Mastering process they brought

up last stream.

Also the next Landscape area planned is on....

Venus!

For the final tidbits this week we got some sort of "Throwback Event" planned which will

grant us past and present day weapon skins like this one for that Lato,

which compared to the current aesthetic of warframe looks friggin' weird!

But curious to see how it pans out.

Someone asked about Riven Transmutations to spend four to get one random one back... which

for some reason Scott thought was too much RNG.

But what are Rivens themselves but RNG incarnate?

Random reward for a random weapon of a random weapon class with random bonuses or even negatives

all of each having random amounts.

But getting rid of four and hopin' for a better one is too random.

Words.

I have none.

Well I got one but it'd be pretty loud XD

Someone asked about adding more appearance tabs which they said might be too stressful to the database,

so take that as you will,

Another asked to make Magnetic damage more useful against enemies as it wipes our energy

entirely and doesn't do anything nearly as debilitating to them.

They sort of shrugged and nodded but it seems even words with Mag in them don't seem to

get any love from the devs now.

The 'Tusk' Grineer of the Plains will stay there, dunno why it was asked as Drekar stay

on Ur....

--planet they come from, you naughty buggers -_^

And both Nef Anyo and the Kingpin system are planned 'sometime after Plains'.

At this point it's a race to see which one comes first, so let's get a poll a-goin'!

Who do you think will we see arrive first?

Nef's rework, the Kingpin system, or Zephyr's Deluxe Skin?

Lemme know what ya think along with anything in the devstream yer lookin' forward to.

If you'd like to see more of these in the future

feel free to subscribe and ring that bell icon for notification squad access and as always

Very elite! Very rare! Very hard to join -_^

And as always, thank you for watching and catch you next time Legion, take care!

WATCH: NFL Star Drops Racism Truth Bomb On CNN… So CNN Cut His Mic | Top Stories Today - Duration: 4:42.

Back in 2014, when the situation in Ferguson was still ongoing, Benjamin Watson, a tight

end for the New Orleans Saints, came home after his game — and witnessed the sheer

madness taking place on his TV.

Over the course of the next day, he had filled up a page on his iPhone notepad expressing

his feelings on the matter.

Watson decided to piece his thoughts together into one Facebook post with an important message.

"It's not about skin.

It's about sin."

He relayed his message, which went viral, and then appeared on CNN to express his point

of view.

He was then rudely cut off.

According to CNN, the post had 427,000 shares, 760,000 likes, and an impressive 78,000 comments.

Watson's message was heard loud and clear by the Christians in this country, regardless

of their skin color.

During his interview with Brooke Baldwin from CNN, he expressed that there were two parts

of his message that resonated with him the most.

The introspection section, and the encouragement section.

In the introspection part, he mentions "How can I look at white skin and make assumptions

but not want assumptions made about me?

That's not right."

He explains that he knows that sometimes you want to "take a side" and call it a day,

but that isn't the right thing to do.

When you make decisions without facts, you are simply giving in to your sin.

Watson goes on in the "encouraged" section of his message to explain exactly how he feels

about that sin, and how it relates to every single one of us.

Watson clearly says that "Ultimately the problem is not a SKIN problem, it is a SIN

problem."

He then explains that sin is the reason why people rebel against authority, it is why

people are racist, it is why people will sometimes lie just to cover for their own race.

Watson goes on to say that, "BUT I'M ENCOURAGED because God has provided a solution for sin

through… his son Jesus and with it, a transformed heart and mind."

This is clearly the kind of message that every American needs to hear right now, particularly

in the wake of the Charlottesville attack.

The NFL superstar brought up the names of some of the people who were the center of

attention a short but unforgettable two years ago.

"The cure for Michael Brown, Trayvon Martin, Tamir Rice, and Eric Garner tragedies is not

education or exposure.

It's the gospel."

In that sense, truer words have never been spoken.

God helps bring us all together, and in this light, even the worst situation produces positive

effects, such as bringing communities closer together than ever before.

This young man's words will ring true, even many years from now.

He is truly ahead of his own time, and understands what communities around the country need to

hear to come together.

We will be saying a prayer for Watson tonight, and we wish for all of the people letting

sin win to finally let it go.

We are all better people than this — regardless of skin tone.

Through God, all things are possible.

My first Q&A video! This week I am answering YOUR questions & you guys had some good ones. - Duration: 14:07.

hello and welcome everybody to my first ever a Q&A video now I'm going to do

this a little bit differently than normal so I have my phone over here for

your guys to be reading your guys comments from I have answers that I've

written down on a book because I had so many so thank you all so much for that I

am also going to be listening to some music and it is going to be the same

music that I am going to be playing in the background of this video because I

figured that right now I don't feel the most high energy and this music really

makes me feel full of energy so without further ado let's get into this what is

your next goal for YouTube so my Mexico for YouTube there's actually very soon

here there's going to be a few changes possibly but I'm going to figure all

that out and then get back to you guys on that because of new things starting

in September so but anyway from my next goal for YouTube is to hopefully start

doing more I feel like I have not been doing as much as I could be and I'm

going to have to change a couple of things in order to get that in working

order properly the way I want it to be so but I'll go over that in more detail

in another video soon hopefully but anyway now wise wizard here says so

what's your goal for future jobs and business and or not to pry but do you

have a future person that you have in your life for companionship here zones

does ask one question sir I'm kidding it does not matter so I wrote my answer

downwards here and do I have a partner right now no I do not

a future partner I don't have any ideas for one but that can easily change I

don't know how that is going to be I don't know it's not really something

that's forefront on my mind right now as for future businesses slash job I

haven't really thought about that too much I really kind of like the idea of

what I'm doing here with YouTube and I definitely want to continue that as much

as possible and as long as possible because I love doing it and I love

meeting all you guys I've met so many new people all of you guys are brand new

in my life and I would have never met you guys if I hadn't started this

channel so I think that's super cool and I definitely want

more of that and to interact with you guys more as for jobs I well I am going

to be teaching parkour against during teaching parkour in September at two

different places so I'm going to end up having what is a total of five classes a

week which is actually really really cool to me and I am super excited for it

I cannot wait sunny fluffy asks I got a question what gets you doing stuff if

you feel motivated but just don't feel up to attempting much there are lots of

times where I feel like I want to do something but I just don't feel like I

have the energy to do it or I don't feel like I'm you know actually wanting to do

certain things and I tend to procrastinate quite a bit and I end up

feeling extremely busy and when I feel extremely busy and stressed I tend to

not do anything and I tend to procrastinate even more which doesn't

help the situation but well usually what I am doing is sometimes I will take a

nap because if I take a nap it kind of clears my mind it settles me down a

little bit and I can focus a little more on what it is I need to do / want to do

for that also sometimes playing a game as I kind of procrastinate a lot so

playing games can end up helping with that or if it is really bugging me and

I'm just like having a really tough time getting started with it a lot of times I

will walk away just completely walk away I'll completely ignore it I'll write it

down or something like that right but I'll just completely walk away and do

nothing do something else for myself and come back to it later on ragin Cajun

Smiths is all right so Nate you're always so cool and collective well I

appreciate that he L that what is your pet peeves it's not showing up on there

it doesn't want to focus that's what is your most annoying pet peeve so my most

annoying pet peeve is when things do not work the way they are intended to work

and I don't mean plans not working out the way I want or certain things that I

want to do not working out how I want it to do I mean items like for example my

phone here because it likes to do stupid things to me

especially with the camera failing garbage right now also you know computer

laptop things like that if even if my chair my chair doesn't want to work the

way I want it to work or something it sets me and I don't know I don't know

why I just going to set when things do not work the way they're supposed to

work because for me if there's no actual reason for it it shouldn't be happening

and I see a lot of times with my phone and laptop problems that shouldn't be

happening because there's no reason for it and I call them ghost issues and I

don't mean that because it's a ghost causing it but I mean it as in it

doesn't exist there's no reason for it to exist it's just a problem out of

nowhere that no one else has Nico asks do you like spoon emoji no it's

completely evil how dare you ask such a thing

and my mother asks do you ever plan to cut your hair I already know the answer

but others may not so my hair my my long long hair my hair is I actually don't

know the measured length of my hair but that is how long it is and yes I do plan

on kind of my hair actually no not cutting I plan on shaving my head at

some point I want to well my original goal was to let my hair grow down to my

butt or past it but it kind of stopped growing at this length because it's been

this way for about a year now I think so yeah that plan is kind of ruined but I

still want to shave it and I want to do some huge gigantic charity event when I

do shave it I don't want to just shave it for the heck of it I wanted to go to

charity I want I want to shave it to raise money for charity I want it to go

to like two three maybe four different charities all at once one of them being

the MS Society oops I just want my mic one of them being the MS Society for

sure skills going to masks what season of the year do you like the most

whoa a team question I know lol but my mother used to say that your favorite

season tells a lot about your person and I kind of believe it so my favorite

season on the way I just left my phone there I need

that my favorite season of the year I wrote it down here is generally spring

because I like the kind of equal mix of cool versus warm days I don't like it

when it's too hot I don't like it when it's too cold I like the snow but I hate

the cold I hate being hot so hot that I end up

sweating but I love those days when the sun is shining and it's windy out and

you have that nice cool breeze but you have the nice light and warmth from the

Sun I also really enjoy fog Omar asks I don't have a question but what is your

favorite song well I actually answered that question the first time I did one

of these question videos because I was the first question I asked you guys

anyway that is okay I really I related to light my favorite song is actually

and still alive by Lisa Murkowski if you don't know that is the same song or main

song I guess for the game Mirror's Edge and I have not played Mirror's Edge yet

though I do plan to but I love that song when I first heard that song that I I

think I had just started doing parkour and it it I answer this in the in the

video about your favorite song right but it reminds me that I am still alive that

I'm not finished yet that I can keep going and no matter what I'm still here

zing cat ass hmm what did you see your life being like in five years from now

well I wrote that one down so that's a little bit more of a complex question so

I said here making a living teaching parkour able to go freely donate to

charity living in a house of our own with my

dream computer still doing YouTube if I'm going to get a dream computer I'm

going to go all out and get the best possible thing I can get but aside from

that still doing YouTube definitely I definitely still want to be doing

YouTube because I love doing this with you guys like I said a hundred times

before and I'll continue saying it forever because it's absolutely the

truth I would also be completely amazed to me if I could continue making a

living or while actually making a living period teaching parkour or doing

something with parkour although I don't really care for competitions that much

but I love teaching it especially to children children love it and it's it's

just I don't know it is it's a cool feeling I love doing it myself but to be

able to share it with others it's it makes it even better and what I said

there about being able to freely donate to charity that's one thing I love to do

i-i've always wanted to be able to do I I have always kind of wanted to be you

know rich I guess but the only reason why I want to have excess money coming

in is so that I can bless others with that money I know what it's like to be

to struggle financially and we still do although not as much and it's not you

know extreme there are people there are tons of people out there who are much

much worse off than me and my mom are but it still sucks struggling

financially like worrying about not being able to go get groceries when you

need to or not being able to pay certain bills or that kind of thing and then

worrying like what's going to happen now if I can't do this and I don't want

anyone else to have to live with that live that way so I would love to be able

to have excess money coming in so that I can help others in that way so that's

where I see myself in five years I was a bit complicated by PS and our blood you

ask what is your favorite game well my favorite game I answered this again in

two weeks ago I believe his sir anyway my favorite game is Beyond Good and Evil

and I don't really understand why but it made some kind of emotional connection

with me and I don't know I just knew that that was a game to kind of stuck

with me I didn't even realize it was my favorite game until I was able to get it

for free it need to ask you already have a nice list of questions I hope you'll

get this which one is where it's not being able to shoot slash make fire when

there are dozens of goats around or being dropped to see it bananas the Wise

Ones know what I'm talking about the goats definitely those goat butts

aren't going to burn themselves and croco svk you ask out what's my favorite

movie series and book also what do you study slash where

do you work well my favorite movie series is definitely terminator slash

The Matrix it's hard to pick between the two but those are the two I love the

absolute most as for a book it is that book that I've talked about I think last

week I still forget the name of it but I think it's starting to come back to me

and I believe it is called quest for something and I want to say quest for

Camelot but that sounds like it's way too I guess mainstream or common or I

don't know what the word is but that might be the name of the book maybe I

can take a quick look up what that is oh my god well looking it up I actually

found the name of the book that I was looking for I looked up the the quest

for Camelot and oddly enough I end up getting this book come up called Grail

quest the Camelot spell that is the book I am talking about that is my favorite

book and I again I don't really know why but I read that book multiple times

throughout school throughout a junior high I believe and it was really really

interesting to me it gave me a very vivid imagination and I still remember

it - the sane I haven't even read any of it in years literally years as for what

do I study I study parkour does does that count I was doing Taekwondo as well

as I haven't this past year though but as for actual school I am NOT in any

school right now any actual school classes or anything right now and I work

teaching parkour to children and I'm going to be having as I said five

classes come September possibly more later on and it's it's extremely

exciting for me and that seems to sum it up for all of the questions for this

week from all of you guys so thank you all so much for your participation in

this I enjoyed it I enjoyed the questions that you guys came up with

so we're simple simple more complex I loved both and I hope my answers are

well enough I guess I did check with me and I didn't see any comments on there

about any questions so that is how that is but

anyway I thank you all so much for watching I hope you enjoyed this video

and I hope you enjoyed the music in the background here might have been a little

bit too dramatic or something and yeah so I will see you all again next week I

did not come up with a question for this week so I think next week I will ask you

a new question because right now it's a it's a late night and I think this

question had a very very large amount of stuff in it if you guys want what you

can do is answer your own questions or pick a question from the comments below

that I did from from what I answer today pick a question and answer it for

yourself and I will read that next week and come up with a new question for you

guys myself but until then thank you all so much watching and I will see you all

again very soon have a wonderful Monday and Godbless

Utilization Management of High-Cost Imaging (September 2017 Radiology) - Duration: 23:08.

[music]

Herbert Y. Kressel, MD Hi. This is Herb Kressel and welcome to the September Radiology podcast.

This morning I'm joined by Dr. Jeffrey Weilburg who is Medical Director of the Physicians

Organization at Massachusetts General Hospital and Associate Professor of Radiology at Harvard

Medical School and Massachusetts General Hospital.

Dr. Weilburg and his colleagues wrote a very provocative article entitled "Outpatient

High Cost Imaging Utilization Management in a Large Stable Patient Provider Cohort Studied

over 7 Years."

Welcome Dr. Weilburg and thanks so much for joining us.

Jeffrey B. Weilburg, MD Thank you.

Now Dr. Weilburg many of those who are watching or listening to the podcast are not that familiar

with how healthcare is organized in the U.S. and how efforts like utilization management

are actually addressed.

Can you tell us a little bit more about your institution, the payer mix and the requirement

that you attempted to address with developing the decision support tool that you use.

Yes, so I do need to correct your introduction.

I'm Associate Professor of Psychiatry.

Oh yes.

Everybody that I meet is a radiologist until proven otherwise.

Yeah, so I think I'm an honorary radiologist.

Okay, perfect.

So this is Mass General Hospital which is a large, urban academic tertiary (inaudible)

medical center located in Boston Massachusetts.

It is part of what we call the Partners Health Care System which includes the Brigham and

Women's Hospital and a bunch of other hospitals throughout the eastern Massachusetts area.

The way we worked it out is that there's the general hospital which is the hospital

and then the physicians organization which includes about 2500 doctors primary care specialty.

Our payer mix is about half – 40% to half commercial payers and the rest government

payers Medicare and Medicaid.

To put it in a nutshell for your viewers, what we've done over the last 10 to 15 years

is we moved from a standard fee for service system to gradually more and more towards

what we call a risk based system which is essentially capitation.

That's very relevant for what happened here because what we found is as we entered risks

we were sharing with the insurance companies the problem that healthcare costs were going

up and we knew that one of the reasons healthcare costs were going up is people were using more

high cost imaging.

So we joined them in a way to figure out how do you get the doctors on the same side as

the insurance company and the government to control healthcare costs.

When you just devolve the cost control to the insurer or the government, they worry

most about cost.

When you include the doctors they start to worry about medical quality as well.

So that's why you want doctors to be in to control costs in a way that collaborates.

So what happened here was that as imaging costs were going up the payers came to us

and said to us 'hey we need to control theses costs.

We'd like these radiology benefit management corporations to get prior authorization every

time a doctor orders a scan.'

And that's where the psychiatry piece comes in.

I had gone through that with psychiatry years before and it had some concerns about it.

So we decided okay we will take risk for imaging, for high cost imaging, and we shared risk

with our payers; but to be able to do that we had to develop a system of utilization

management which meant the doctor wanted a CT scan of the head.

Why do they want it?

Is it appropriate?

And then how do you manage that to say yes you can do or um we don't know that you

should do it, maybe do something else.

Does that answer the question?

Yeah.

Now just to drill down a little bit more, in terms of the way the institution is organized

to actively manage the care; at Beth Israel Deaconess I remember we had these primary

care pods.

So rather than having several hundred doctors sharing risks together they were in smaller

groups and they would meet and exchange ideas and look over reports and how is it done at

Mass General?

Is it done centrally or?

It's a very good question.

It's evolved because in the early days of capitation the individual primary care doctors

were themselves at risk which meant that they were worried about high cost cases and spending

because it affected their personal income, and that was too much of an incentive to control

costs because they had to also provide care for the patient no matter what.

In the interim period that this study took place the MGPO and the MGH, the physician's

organization at the hospital, were the risk bearing agents.

So we diffused the risks across all of the primary care doctors and the specialists shared

in the risk.

Right now the risk has moved more centrally even to partners and a little bit further

away from the institution although we have what's called an internal performance framework

where we give imaging data to every single doctor in the institution, because unless

we do and they can keep going up and up and up and when there are outliers we say hey

we're concerned about your performance can you look at it.

So now partners is holding the risk but we end up being very concerned about it.

And then where does imaging utilization fit in the context of overall utilization management?

Is it a major focus, a fraction of it, or…

The big ticket items now for healthcare spending are inpatient admissions and emergency room

and often surgery.

So in our system imaging, high cost imaging, is maybe 6 to 7% of the total spent, but what

the paper showed and what's been very valued at the institution is we brought it down from

9% to around 7% and that has been very important in helping us perform well under a risk based

contract.

Good.

So who at the institution developed the decision support tool and how were the recommendations

determined?

So the – what we started out in 2005 and…

Is that why they hired a psychiatrist to manage the physician pool?

That's actually how it happened.

The radiology order entry commonly known here as ROE was developed by Dan Rosenthal and

Keith Dreyer and Dr. Jim Thrall as a way to do structured better entry and move from fax,

but when the payers approached us and we came back to them and said hey we'd like to it

ourselves, they've reached out to me and I've been working very closely with them

on adding the decision support component to their already built system.

What they did was they had lit by Dan and the other radiologists because I don't know

radiology content.

My job has been interacting with the doctors about this and building decision support.

We developed internally a set of rules based on our best guess of evidence.

Then as you well know the American College of Radiology used some of those rules, but

has developed its own products ACR Select and other things like that and done I think

a pretty good job of looking on a national level engaging experts in particular areas

as well as primary care to develop a pretty open evidence based set of rules which now

cover a pretty substantial portion of all of the indications and orderables.

So there is a pretty good set, there are other competing things with ACR that have gone through

a similar process nationally.

So you largely are using the ACR guidelines and then you modify them as appropriate for

the institution?

Actually the other way around.

What happened in this paper is during the time course of this paper we were using our

internal system called ROE.

Since that time we've switched to Epic and Epic uses this ACR select which incorporated

some of the things of ROE.

So that brings us to the paper.

Why did you decide to do the retrospective review now?

I wanted to keep my job.

Hey we're going to invite you back you keep up the humor.

We put a lot of time and energy into imaging management and I wanted to know were we effective

in what we did.

That's really the straight and honest answer.

And do you know it was an important question from a health policy point of view because

we wanted to know could we take this on ourselves?

Could we manage risk?

Could doctors and the healthcare system be the guardians of the cost as well as the guardians

of the quality and utilization or did we need to have it done by some external agencies.

So if you could show it for imaging then can you do it for other areas like labs and all

sorts of other resource use?

Because no offense to the radiologists, but from a medical director's point of view,

imaging is just one of the things the doctors do.

In my opinion it's absolutely a core piece of diagnosis and treatment and that's why

I didn't want them to carve it out, I wanted us to be directing it based on real medical

evidence.

Good.

Getting to the details of your study, in your study you looked at the utilization of a group

that you called a loyalty cohort of primary care physicians, can you tell us what that

means and what fraction of the total pool of primary care doctors does this represent

and do you get like a merit badge if you're a loyal?

I'm afraid that's not how it works at MGH.

But the loyalty cohort was something developed by one of our lead primary care doctors, Dr.

Stephen Atlas.

He developed it because about 12 to 15 years ago we were trying to get our hands on utilization

overall and we were using the primary care doctor that was designated by the payer, but

when you would show them their imaging they would say to me, what this is not good data.

You're showing me imaging on patients I've never seen.

I didn't order that stuff.

How can you hold me responsible for that?

So the PCPs got together and said let's look at a way of finding the patients that

the PCPs themselves really do take care of; they order imaging, they order labs, they

prescribe drugs, they have visits.

He developed this incredibly reliable way to say doctor these are your patients, this

is your patient panel.

It's essentially 100% to the PCPs at MGH.

Okay.

Well it seems like that's a very important group to identify appropriately for this type

of effort.

Going back to the methods, you sort of as a figure of merit, you looked at the utilization

rates of high cost imaging compared to laboratory utilization and what is the expected relationship

between these two.

Did you have any information about that when you made that decision?

Well no and the humble and direct answer is that this was a retrospective study and we

were trying to do our best to use retrospective analysis to establish as much of a causal

relationship as one could in such studies because it's really mostly just correlational.

We knew that the other research in the area had some correlation between imaging UM and

utilization.

But what we asked is if – I joked with you before about imaging being a resource from

a medical director's point of view, labs is another resource.

The reason we picked labs is the doctors at that point they knew the unit cost of an image

tended to be higher than the unit cost of a lab, but it's not always true if you order

a genetic panel it's many thousands of dollars.

So they tended to see these things as diagnostic tests that they would order and manage for

patients.

The imaging was very intensely managed.

Laboratory at that time was not managed, so in some way there's a rough comparison.

And in the paper we discussed it's at best a rough comparison, but when the paper focuses

on the probability that a doctor will order a scan.

We measure propensity, probability to order and the intensity of the number they order

when they do measure.

The probability that the doctor is going to order the imaging is the thing that is probably

most subject to utilization management because that's under the doctor's control, do

I do it or not.

So when you look at the probability that the doctor is going to order imaging versus the

probability they're going to order a lab on the same panel of patients.

The labs went down because we were trying to control costs, we had choosing wisely,

and lots of other things.

People may know there was a recession in this country and patients were more concerned about

utilization.

But imaging went down nearly twice as much and it was a statistically significant difference

and we thought that suggests to us that it was a differential impact on the doctor behavior

of utilization management.

In fact the doctors tell us that.

Can you highlight some of your other key findings in the study that people listening may have

not read the article yet?

It's pretty straight forward.

We found that the total utilization of imaging across our system when down significantly.

I think it was 21 point something percent.

And that the imaging attributed to the primary care doctors, the stuff they ordered for their

patients, went down like 36% whereas the specialists ordering on those same patients also went

down, but a little bit less.

That made sense to us because the utilization management efforts were mostly focused on

the primary care doctors.

It's really much harder to tell the orthopedic surgeon and the neurosurgeon or the endocrine

specialist what to do about imaging because they do a narrower range of things and they

know more exactly what they want when they order an imaging, whereas the primary care

doctors often value the help because they have to cover this wide range of disorders

and they're less expert at any particular thing.

So it made sense that imaging went down for PCPs more.

We found that there didn't seem to be any dumping that we didn't go from the PCPs

ordering it to the specialist ordering it.

And then when we compared it internally to the labs and then we were fortunate enough

to get some local data, we found that even in eastern Massachusetts imaging utilization

went down during the same time period, but imaging utilization here went down substantially

more and we thought hmm they're using these radiology benefit management companies outside

of us, we're using our own internal system.

Maybe that suggests that the internal system is working pretty well.

I see.

Now you eluded to this that as you shifted to Epic the decision support tool actually

changed, but is there a structure for managing this going forward.

There was lot of changes in imaging technology, so are you going to let Epic decide this or

the college or is this still going to be an "autonomous" effort?

So it's a very difficult question to answer.

I think that the understanding of clinical decision support and utilization management

for imaging and many other areas is evolving rapidly.

I think that the – what we're seeing now is Epic is using a company called National

Decision Support Company to provide a ROE like experience but it's very different

in different institutions and work is ongoing to improve it.

I think that the ACR is doing a good job in trying to provide a transparent set of clinical

decision support recommendations that anyone in the United States, doctor or patient, can

see what they're getting and what the evidence is.

I think that kind of thing will continue to grow.

I think we're learning in answer to your question that it's in imaging and other

areas it's not just the clinical decision support, it's not just Epic, it's about

how you engage the doctors and how you make the system useful for them and how you give

them feedback about how they're doing so they can learn.

The problem with the RBMs is it's an external group that says yes you can, no you can't.

What we did is we said yes you can even if you get a score, a red score, that says decision

support says it's not likely to be that useful, you can still go ahead but we're

gonna count up your red exams and we're going to feed that back to you and show it

to you publically, can you explain to us the utility of this?

When you do that doctors are pretty data driven.

They start to understand maybe I'm a little bit off the reservation if I'm much higher

than a like group of colleagues, why is that, and am I getting value.

The best use of clinical decision support is when it becomes a teaching experience for

the doctors.

I'll say one other thing that some of our patients have really liked it because when

a doctor can say there's a set of evidence that helped guide me about whether an image

is useful for you and I'm not ordering this image for your low back pain because I think

it may not be helpful and here let me turn the screen around and show you what the system

says, the patients love that.

If they understand the doctors that try and do the best evidence based system thing for

them rather than just skimping them and not doing it.

I have one last question for you.

I'm sorry to but one thing that you said just sort of struck a chord.

You know we publish a lot of diagnostic accuracy studies and one of things that we are increasingly

requiring is to have multiple readers or multiple institutions because the performance results

of an exam are not just driven by the machine the protocols that we use alike…

Absolutely.

So the question is where is the interface between generalized recommendations and your

own abilities at your local institution in your population with the prevalence of disease

that you're seeing and I think at Mass General you have your arms around that locally, but

I think for people looking at this do you have any suggestions as to how they deal with

that issue?

Boy that's going a little bit beyond the paper.

Our department of radiology let me see if this is on track to what you're saying.

It's really interesting because we've been looking at what we call leakage, is imaging

– is there a value in the imaging done within our system where we pay very close attention

to the protocols and the technologists and how they do the images and the quality of

the imaging versus something that's done in another system and sometimes it's just

as good, but sometimes it's not and then do you have to repeat the scan and is that

a cost issue and a quality issue, so we've tried to steer things we think towards our

own internal system to control leakage because of the protocol and interpretation.

The other thing is there's a big issue about radiologist's recommendations.

We looked at how much of imaging is driven by recommendations made by the radiologists…

In the reports.

…a sort of an urban myth among the primary care doctors that the radiologist

made me do it.

It turns out that when you look at it it's not as big as you would think; it's less

than 10% and declining.

We also looked at, and other systems may want to do this we had fun with this, we looked

at profiling our radiologists by their recommendation rate, repeat or follow-up imaging, and we

found that not surprisingly there was a direct correlation with the tendency to suggest repeat

or follow-up imaging with the experience level of the radiologist.

The very senior, very experienced radiologists were more comfortable with their ability to

interpret the image.

We have not completed work on how accurate they are, but that would be a next step.

It is hubris or is it true experience.

Dr. Weilburg I really want to thank you for spending the time and sharing your insights

on this fascinating paper and we look forward to more papers on your work.

Thank you very much.

It's a pleasure to talk with you.

Thanks for joining us.

Richard Dawkins: The Dangers of CRISPR, Designer Babies, and Artificial Genetic Mutation - Duration: 4:08.

So I don't regard myself as a biological evolutionist as qualified to talk about technological

evolution.

I'm interested in the analogy but I think it only is an analogy and it doesn't go

too far.

Filmmaking in 7 Minutes – Filmmaking 101 - Duration: 6:43.

Welcome to Echo productions, I'm Silas Willoughby and in today's short video I'm going to

try and teach you everything you need to know about filmmaking to get started.

Let's start with the most basic thing in filmmaking, types of shots.

This Is an extreme wide shot, or establishing shot, it makes the characters look small,

Extreme wide shots are commonly used as establishing shots, to show were the character is in the

scene, and to establish how the scene is laid out.

Establishing shots don't have to show people, they can just be of the area, or the outside

of the building that the character is in.

This is a wide shot, wide shots are used to establish the charter in the scene, you get

a full shot of the actor, but you can still see a lot of the background and what's going

on around them.

Wide shots are often used to give clarity to what's happening in the scene, so the

audience doesn't get completely lost in the action and tighter shots.

This is a full shot, it shows the actor fully, from head to toe.

This is a medium shot, or a mid-shot.

Medium shots tend to be the most common in almost every film, a medium shot frames your

actor right above their waste and leaves a little bit of space above the top of their

heads.

This is where we start to having to make important decisions about where the camera goes, and

what it shows.

This a medium close up, sometimes called bust shot, generally framed from the chest to the

top of the head.

This is a close up.

These generally focus on the face of you actor.

This an Extreme close up, it focusses on one aspect of the face, most commonly the eyes,

but also the lips, or mouth.

Or really any other part of the body you want to make the audience focus on, your not really

giving the audience a choice of what to focus on at this point.

This is an insert shot, these don't focus on the people, but are used to emphasize something

that is important to the story.

It could be someone gripping their gun tightly to show nervousness or a secret code, or anything

really, often its used to help the audience realize what's going on before the character

so they can be more concerned about the character and more emotional invested in the character.

The insert shot or the cut away shot can be used for things like cutting back and forth

between two characters in other locations, like a phone call for instance.

This is an over the shoulder shot, this is when you place the camera over the shoulder

of an actor, this allows you to see the character and whatever he or she Is looking at or talking

to.

Over the shoulder shots are most often used for conversations.

Over the shoulder shots can be anything from wide shots to close ups, it's up to you,

but generally they are used for mid shots.

Framing like this is called the rule of thirds, generally you place your character on the

vertical lines.

Generally, your character should have their eyes on an intersection between two lines.

I made a short two-minute video about this a while ago if you want to check it out.

The last rule you need to know about framing is called looking space, you do this by framing

your actor on the opposite vertical line than they are looking at this feels a lot more

natural than putting them on the other side.

You can frame them on the other side, stylishly but its complicated so I'm not going to

be explain right now.

And finally, there is a Dutch angle, this were you put the camera at an angle, and is

often used to show that a charter is unhinged, or to make the audience un easy.

The movie Thor uses a Dutch angle a lot.

But that's about it for framing, now let's look at camera movement.

This Is a pan shot, it the most common and natural movement you can make with your camera

because it looks the same as looking around does .

This a tilt shot, it's basically the same as a pan shot just vertical instead of horizontal.

This is a Dolly, a dolly is when you move the camera perfectly smoothly towards or away

from something.

This is a zoom, it when you Zoom in on you lens or in post.

This is a tracking shot, its when you track with the actor, either behind them, in front

of them or from the side.

This is a crane, or a jib shot, it's when you raise the camera vertically, no to be

confused with a tilt which simply pans upwards.

But that's all the basic types of camera movements now let's move on to lighting.

Now lighting is a very complex subject, I even have a 7 minute video explaining just

the basics of lighting, lighting is the one thing that gets the most overlooked by beginners,

it's the thing that can make your film go from being amerture to pro looking.

Now the most basic of lighting techniques has three lights, a fill light a key light,

and back light.

The key light tis the main light in the scene and is generally the brightest.

The fill light is generally placed opposite the key light and is used to regulate the

amount of shadows on the subject.

And the backlight is used to separate the subject from the background by rim lighting

them.

High and low-key lighting.

High key lighting is a brighter fill light and used mostly in comedies, and low-key lighting

is less or no fill light.

Low key lighting is used most often in horror or crime thrillers.

But you can do pretty much anything with lighting to create the specific mood you are looking

for.

That's pretty much it for lighting, so let's move on to another often overlooked and very

important subject, sound.

Sound is one of the most overlooked things in filmmaking, People often say that sound

Is more important than the visuals in a film, Because Audiences will tolerate bad pictures,

much more then bad sound.

There are two main types of mic used for filmmaking, Shotgun Mics and lavalier mics.

Shot gun mics are the most commonly used mic in filmmaking.

The reason shot gun mics are used so much is because they pic up sounds from the front

and not from the side or the back, this allows you to record the sounds you want, and not

unwanted sounds like planes overhead or cars in the background.

Most professional shot gun mic look like this, however you can also get mics that look this,

these are great and really simple because all you have to do is plug them in your camera,

this type of shot gun mic is also a lot cheaper.

In fact I made a video about using this type of shot gun mic to get clean audio for only

30 bucks! when using a pro shot gun mic you generally use and external recorder and sync

the audio up in post to get the cleanest sound possible.

Shot gun mics are commonly used on boom poles, to allow you to get closer to the source of

the sound without getting in the shot.

Lav mics or lavalier mics, are often used for interviews or back up audio since they

are very small and can clip on shirt collars or ties.

However, the sound quality isn't as good a shot gun mic, so when you can its best to

use a shot gun mic.

Ok, that's sound done, now let's move onto editing.

The simplest edit you can do is the cut.

Simply cutting between two shots, in a sequential manner.

The most natural cuts cut on action.

This means cutting from one shot to another while the subject is still in motion.

These are sometimes called match cuts by amateurs because you are keeping the movement fluid

between the two shots.

These cuts occur most often from things like kicks and punches, and people going through

doors.

This is a jump cut, a jump cup is often used to show the passing of time, or to create

a feeling of urgency to the scene.

Jump cuts are created by cutting between the same shot.

Next there's the cut away, which is basically cutting to an insert shot.

Now let's move on to transitions This is a dissolve, that's all there is

to say about that This is a fade in or fade out, basically a

fade to or from black.

This is a wipe, There's lots of these in star wars.

Well guys I think that just about covers all the basics of filmmaking so you can get started.

No Members Of Trump's White House Will Go On TV To Defend Him, Not Even On Fox - Duration: 3:29.

Typically on Monday morning, and even Sunday afternoon, most of the news is buzzing with

some of the crazier things that the Trump Administration officials said during their

appearances on the Sunday news talk shows, but this week was a little bit different.

We had no crazy statements to pick apart.

We didn't see Kellyanne Conway's face grace our television screens.

In fact, not one single member of the Trump Administration went on any of the Sunday talk

shows, and that includes not going on any FOX News programs.

According to FOX News themselves, they could not find a single official from the Trump

White House willing to come on their network to talk about the President's really bad week.

Keep in mind, a few days ago, FOX News had to come out and say, "Look, we couldn't even

find any regular Republicans willing to come on and defend the President."

Now, they can't even get members of the Trump Administration to come on and defend him?

Look, I understand if they don't want to send people to go talk with George Stephenopoulos

or Chuck Todd, but not even FOX News?

How bad are things behind the scenes when you won't even go talk to your friends that

helped put you in office?

You know FOX News isn't going to ask you any tough questions.

You know they're probably not going to ask about any of the horrible things that happened

in the last week.

Yet, the Administration is still too afraid to even send anybody in front of those cameras.

Things are pretty bad in the White House, and I think that is a very gross understatement

at this point.

When they're completely unwilling to go out there and spew their lies and hatred like

they've done every single week since they came to power, you know something has devolved

into something even worse behind the scenes.

Donald Trump did arrive back in DC on Sunday evening, so he wasn't even there Sunday morning,

technically, to prevent anyone from going on TV.

I'm sure he issued some kind of order, maybe there's a memo that's going to surface later

on this week, where they said, "For the love of God, don't go out and talk to the media."

Maybe it was General Kelly, Donald Trump's new chief of staff, trying to do some damage

control and preventing these idiots from going on television and actually making things worse.

Because at this point, that's all those people can do.

Sarah Huckabee Sanders, Kellyanne Conway, Stephen Miller, Sebastion Gorka, there is

nothing they could say at this particular moment that would make anything that Trump

has done in the past week seem better.

All they can do is add fuel to that fire, which, of course, the rest of the media, the

blog world, is going to jump on and absolutely use it to dismantle this administration.

So it was probably a good call that they decided to withhold their people from the Sunday talk

shows but, unfortunately for them, being such a vocal administration, when they finally

go silent, you know something's gone horribly wrong.
